Title: CDPAS Home Healthcare Program Specialist

Full Time, 40 Hours Per week, M-F 9:00AM-5:00PM

Location: Watertown, NY

Program: Consumer Directed Personal Attendant Services

Anticipated Start Date: Late February/Early March 2018

Summary:CDPAS Specialist helps with the administration of a non-traditional home healthcare program in Jefferson and Lewis Counties. The Specialist will assist with employment paperwork, orienting new employees, scheduling, maintaining consumer and attendant files, assemble documentation packets, and respond to inquiries in a timely and proficient manner.

Requirements:Associate’s degree preferred. GED/High school diploma with related work experience required. This position will require the ability and willingness to travel frequently, organizational skills, timeliness, proficiency in data entry and typing, research, and computer skills, excellent interpersonal and communication skills.

Title: NY Connects Coordinator

Full Time, 40 Hours Per week, M-F 9:00AM-5:00PM

Location: Ogdensburg, NY

Program: NY Choices in Long Term Care – NY Connects

Anticipated Start Date: Late February/Early March 2018

Summary: The NY Connects Coordinator assists individuals with disabilities, chronic health conditions, the elderly, and their family/friends to learn about long term care services and community supports that will help the person remain independent. The Coordinator will educate on available resources, link the consumer with other providers, provide enrollment and application assistance, and support. The Coordinator will serve on advisory boards, attend community meetings, and provide public outreach events and public speaking presentations. The Coordinator will provide services to consumers at various locations throughout St. Lawrence County and will be required to meet consumers in their homes, as needed.

Requirements: Bachelor’s degree, ability and willingness to travel frequently, organizational skills, timeliness, data entry, research, and computer skills, excellent interpersonal and communication skills.

Title: Independent Living Specialist & Advocate

Full Time, 40 Hours Per week, M-F 9:00AM-5:00PM

Program: Independent Living Services

Anticipated Start Date: Late February/Early March 2018

Summary: The IL Specialist assists individuals with disabilities and chronic health conditions to obtain support services in the community to remain independent in the community. Services include, but are not limited to: benefits advisement, application assistance, advocacy, housing assistance, peer advocacy, independent living skills training, employment, and information and referral.
